• Customer or company name and contact number
• Order or invoice number and purchase date
• Product name, brand, and model
• Serial number, if applicable, and a copy of your proof of purchase

Our team will verify the purchase and advise on the available warranty documentation. Please do not include patient information or unrelated personal records.
